この症例報告は,重度の認知障害により日常生活の基本動作ができない高齢者が,入院という環境の変化に適応していく過程をまとめたものである。本研究の目的は,看護者が実践する基本的な生活援助が,痴呆性高齢者の行動や感情に与えた効果について明らかにすることである。対象は老年期痴呆の80歳代女性Yさんである。生活援助特に食事の摂取状況とアクティビティ活動から分析した。その結果,彼女は食事を分割する介助方法によって,自力摂取することが可能となった。また,塗り絵にも積極的に関わるようになった。彼女は入院によって家族から分離させられ,分離不安を起こしていたが,看護(介護)の援助を通して,彼女は家族の代理として,看護師や他のスタッフと良い人間関係を保つことができるようになった。基本的な生活援助の継続は,痴呆性高齢者に残されている可能性を引き出し,痴呆症の経過の遅延に影響を与えることがこの事例から明らかになった。
(英文抄録：Present study described the process of environmental adaptation during the entering a hospital in the patient with severe cognitive disorder who unable to daily action alone. The purpose of this study is to examine the effect of basic nursing care on the daily activities and psychological status in demented elderly. The object of this study is a woman in 80-year-old with senile dementia and assessed using rating scales of ADL (Ability of Daily Life) and activities. As the result, it became possible that she took a meal herself by assistance method for dividing the meal, and it would be also engaged in activities such as a line drawing for coloring. Her psychological status became high anxiety when separated from her family. However, she could keep good human relations by humanistic nursing intervention. This study suggested that the continuation of the fundamental life leads to increased ADL and activities in the demented elderly.)
