Abstract

 血液透析は末期腎不全に対して体液異常の是正,病因物質の除去を目的に実施される体外循環治療である。透析患者では様々な合併症が起こる。特に透析後の起立性低血圧は患者の生命予後に対する危険因子や,日常生活活動に対する制限因子となる。本研究の目的は,血液透析後に発生する起立性低血圧に対する運動療法介入の効果を評価することで,高齢血液透析患者に対する運動療法の継続が透析後の起立性低血圧に与える影響について明らかにすることである。対象は慢性腎不全と診断され,外来にて血液透析中に運動療法介入を行っている慢性腎不全患者9名とした。運動療法は下肢のストレッチングや筋力増強運動を行った。評価は透析終了直後に血圧測定と脈拍測定を行い,背臥位から頭部挙上位の収縮期血圧を引いた値を起立時低下血圧とした。評価は2回行い,1回目と2回目の間は3か月間とした。起立時低下血圧の結果は,低下幅が減少する傾向にあった。今後,本研究結果をもとに評価項目や介入内容を検討していく必要がある。

Hemodialysis is an extracorporeal circulatory treatment to correct fluid abnormalities and remove pathogens in patients with end-stage renal failure. Patients undergoing dialysis develop various complications; specifically, occur in dialysis patients. In particular, orthostatic hypotension after dialysis is associated with a risk factor for a patient’s life poor prognosis and a limits factor for patients’ activities of daily living. The purpose of this study was to evaluate the effect of exercise therapy intervention on orthostatic hypotension occurring after hemodialysis, and to clarify the effect of continuing exercise therapy on post-dialysis orthostatic hypotension in elderly hemodialysis patients. The study included subjects were 9 patients with chronic renal failure who had been diagnosed with chronic renal failure and who underwent exercise therapy, intervention including lower limb stretching and muscle strengthening exercises during hemodialysis on an outpatient basis. Exercise therapy included lower limb stretching and muscle strengthening exercises. The blood pressure and pulse measurement and pulse measurement rate were recorded performed immediately after the end of dialysis, and the value obtained by subtracting the systolic blood pressure of the head elevation from the supine position was defined as the orthostatic falling blood pressure reading at standing. Blood pressure the evaluation was measured performed twice, and the period with a 3-month interval between the first and the second evaluation was three months. The results of standing-down hypotension tended to decrease. In the future studies are warranted, it is necessary to examine the evaluate ion the items and intervention contents based on the results of this study.
