Abstract |
介護老人福祉施設の計画担当介護支援専門員の介護支援業務を明らかにし,その標準化と専門性について検討した。専任計画担当介護支援専門員342 人と他職種兼務者457 人の連続する2 日間の自計式タイムスタディ調査から,業務内容を業務大分類16 項目と業務中分類110 項目からなるレジデンシャル・ソーシャルワークコードに置換し統計解析を行った。専任計画担当介護支援専門員は,「アセスメント」,「ケアプラン」,「チームマネジメント」を回数,時間ともに他職種兼務者よりも有意に多く実施していた。他職種兼務者は,4 割以上が介護職を兼務しているために,「ケアワーク」を専任計画担当介護支援専門員よりも有意に多く実施していた。業務量から捉えた専任計画担当介護支援専門員の業務は,チームマネジメントによる施設ケアマネジメントを主業務として実施していた。計画担当介護支援専門員の専任化が標準となり,施設ケアマネジメントの専門性を高めていく必要があると考えられた。 The purpose of this study was to examine the specialty of care managers in geriatric welfare facilities. We conducted a self-report time study of services provided to residents of geriatric welfare facilities for two consecutive days. The study measured the time full-time care managers (n=342) and part-time care managers who hold the dual positions (n=457) spend in caring for their residents. Statistical analysis was performed after the recorded work contents were replaced with the Residential Social Work Codes consisting of a large group of 16 items and a medium group of 110 items. It was revealed that for the full-time care managers, the most common activities were "assessment," "creation of care plans" and "team management." As for the part-time care managers, "care work (direct care)" was the most common activity since more than 40%of them hold the dual positions of care manager and nursing assistant. The results suggest that the major role of the full-time care managers is to participate in a team approach to create care plans for the residents and this may be considered as an expertise skill for the care managers in geriatric welfare facilities in Japan. |
