本研究は, 医療機関における専門職のチーム医療に対する認識について, 職種および職位による違いを明らかにすることにより, チーム医療を推進していく上での問題点や課題を明確にすることを目的として, 平成14年11月から12月に調査票を用いて留め置き法による郵送調査を行った。調査施設はH県内の医療機関で, 一般病床数200床以上の病院29施設と機能評価認定病院17施設の46施設である。調査対象者は医師, 看護師, 理学療法士, 作業療法士, 言語聴覚士である。30病院 (回収率65.2%) から協力が得られ, 回答者数は1261人, 有効回答は1205人であった。「チーム医療に対する考え方」の25項目の平均点については, リハビリテーション専門職は協働意識, アセスメントなどの18項目で他の職種より高く, 医師は, リーダーシップ, 看護職は, チーム医療による仕事の増加の負担感が高くなっていた。「チーム医療に対する考え方」の因子得点は, 因子1「協働意識」, 因子2「必要条件」, 因子6「職種独立性」においてリハビリテーション専門職が有意に高く, 因子3の「自己啓発」は医師が高く, 因子5の「業務負担」は看護職が高くなっていた。「チーム医療に対する考え方」において, 「チーム医療の実践の可能性」はスタッフが管理職より少なく, 管理職では「チーム医療の中でリーダーシップが取れる」が低くなっていた。チーム医療に対する考え方の職種, 職位の違いから, リーダーシップ能力の向上, 業務の負担感の軽減が課題であることが明らかになった。

This research is to clarify that there is a difference in the way team care recognition is regarded by people in different occupations and professional grade of the health care persons concerned. We mailed a questionnaire to review the problem to advance team medical care in Japan. We surveyed physicians, nurses, physical therapists (PT), occupational therapists (OT), and speech language hearing therapists (ST) and managers in 46 of hospitals of H. Prefecture. The hospitals included 17 general hospitals with 200 beds and 29 specialized hospitals. We received replies from 30 hospitals (65.2%), which resulted in 1205 effective replies out of the total 1261. We compared the average scores of 25 items on "the difference in the recognition of medical team treatment". As for the scores from PT, OT and ST, 18 items including "collaboration", "assessment" etc., were higher than for the other health care professionals. Physicians scored high for "leadership" ; nurses responded that the need for "team care" is high, although there was some disagreement. We divided "the concept of team medical treatment" into 6 factors. Factor 1, "collaboration" ; factor 2, "the necessary conditions" ; and factor 6, "independence" were scored high by PT, OT and ST. Factor 3, "self enlightenment", was scored high by physicians. Factor 5 "the business burden", was score high by the nurses. For, "the concept of team care" and "the possibility of team medical care", the professional staff scored lower than did the managers. Half of the managers responded that leadership "can promote" team care. We found that the perception of team medical care differs by occupation, and grade. We suggest that the team approach can induce improvement and reduce the burden of the leadership in hospitals.
