外来で化学療法を受けながら生活するがん患者の困難と対処を明らかにすることを目的に半構成的質問紙を用いた面接を行ない,内容分析を行った。対象者は,8名の女性乳がん患者で,研究の同意が得られた者であった。外来で化学療法を受けながら生活するがん患者の困難として,<通院で化学療法を受けることから生じる負担><化学療法の副作用による辛さ><取り除くことができない症状の辛さ><今より状態が悪化することへの懸念><自分らしく生きることへの揺らぎ><がんと共に生きることの脅威><死を意識する辛さ><治療による経済的負担><医師との協調での戸惑い>の9つのカテゴリーが抽出された。対象者は多くの困難を抱え対処していたが,適切に対処していない困難もあった。看護者は,患者がこれらの困難を克服し,治療を継続しながら患者が望むような生活を維持できるよう支援をしていくことが必要である。

The purpose of this study was to clarify the difficulties of cancer patients receiving outpatient chemotherapy and their management. The subjects were patients with cancer who consented to the study, while receiving outpatient chemotherapy. We interviewed the subjects using a semi-structured questionnaire. Using content analysis, we divided the difficulties of cancer patients receiving outpatient chemotherapy and their management into several categories. The subjects were 8 females with recurrent or metastases of breast cancer. The mean age was 53.3 years. The difficulties could be classified into 9 categories; 1) burdens of receiving outpatient chemotherapy, 2) suffering from side effects of chemotherapy, 3) suffering from conditions that cannot be palliated, 4) threat of exacerbation of the cancer, 5) anxiety over the inability to control their own lives, 6) fear of living with cancer, 7) distress of the consciousness of death, 8) economic burden, and 9) confusion on cooperating with the doctor. And the subjects managed most of these difficulties individually. Although cancer patients receiving outpatient chemotherapy had many difficulties, they managed to overcome these difficulties individually. Therefore, nurses must give them support to help them overcome these difficulties and live their lives as they wish to, while continuing outpatient chemotherapy.
