乳幼児健康診査は,障害の早期発見とその後の支援と連携に大きな役割を果たしている。今回,広島県東部3市における乳幼児健康診査後の支援体制と連携について,乳幼児支援と連携に関する内容調査と支援と連携の現状に関して担当保健師に対する聞き取り調査を行った。結果,乳幼児健康診査後の発育・発達相談は,発達の遅れの受容などの問題があり,保護者にとって利用しにくいという認識もある,相談が必要であっても顕在化していない児も多いことなどから,育児・発達相談の必要性に対する啓蒙が重要であり,また各種専門家は支援によって得られた児や保護者の満足感や支援の有効性を示す必要があると考える。さらに,5・6歳の就学前児の相談が増えてきている市もあることから,就学に向けた支援を必要とする児の発見のために時間軸に沿った縦の連携に対する対策なども必要であると考える。

The public health center plays a key role in infant check-ups and in the early detection and follow-up for children with signs of possible developmental delay. The purpose of this research was to investigate the structure of the support systems for following the development of these infants after the check-ups. The public health nurse was the main liaison for the support systems in our research. As a result, we found that enlightenment to the necessity of support and follow-ups, including consultation services, is particularly important. In addition, the specialists involved should provide proof of the effectiveness of the support system and satisfaction of the families involved. Moreover, longitudinal studies of the follow-ups are also necessary, because the need for follow-ups for preschool children 5 to 6 years of age has increased. In some communities, development of such programs is already beginning.
