Abstract

本研究は,産褥早期における母乳哺育支援のために,産褥3日目における母乳栄養の状況と母乳中の分泌型IgA(sIgA)濃度との関連を明らかにすることを目的とした。正常な妊娠経過をたどり正期産で経膣分娩に至った61名を対象とした。また,心身の状態を知る目的から母乳採取と同時にPOMSの測定を行った。その結果,次のことが明らかになった。①新生児の哺乳量のうち母乳の占める割合(母乳割合)と母乳中sIgA濃度間に負の相関を認めた。②母乳割合は経産婦と比較し初産婦の方が有意に低かった。③POMSの得点によって母乳中sIgA濃度に差はなかった。初産婦経産婦を問わず母乳の哺乳量が少なくても,新生児に免疫上の益を効果的に与えていると推測される。したがって,少ないからとあきらめないで母親を励まし支えるという母親へのサポートのための根拠が得られたと考える。

The object of this study was to clarify the relationship between the concentration of secretory IgA (sIgA) in breast milk on the third puerperal day and the nutritional benefit for breastfeeding in the early puerperal period. The test subjects were 61 women who had a vaginal delivery at full term after a normal pregnancy. Also, in order to understand the physical and mental state of the subjects, when the breast milk samples were taken, a shortened version of the Profile of Mood States (POMS) was also administered. From the analysis, the following points became evident. 1) There was a negative correlation between the amount of breast milk fed to the infants and the concentration of sIgA in the breast milk. 2) The amount of breast milk for the primipara women was significantly lower than that of the multipara women. 3) There was no correlation between the concentrations of sIgA in the breast milk and the results of the POMS scores. The results suggest that the mother's milk can effectively provide an immunological benefit to the newborn baby even when there is a small amount of breast milk. Therefore, it would be advantageous to encourage and support mothers with insufficient breast milk.
