本研究の目的は，外来化学療法を継続する進行肺がん患者の抱える問題を明らかにし，看護援助の方向性の示唆を得ることである。外来化学療法を2クール以上継続する進行肺がん患者7名を対象に，半構成的質問紙を用いて面接を行い，質的帰納的に分析を行った。外来化学療法を継続する進行肺がん患者の抱える問題は，【抗がん剤の有害事象の症状による苦痛】【身体症状の悪化による日常生活への支障】【化学療法の不確かさによる不安】【がんの進行に伴う不安】【人生の終焉のありようへの不安】【外来で化学療法を継続する難しさ】【社会的役割を果たす難しさ】の7つのカテゴリーが抽出された。外来化学療法を継続する進行肺がん患者への看護援助として，1) 持続する抗がん剤の有害事象による身体的苦痛を緩和する2）不確かな治療を継続するための不安に対する精神的サポートを行う3）外来化学療法を継続するために患者を取り巻く環境を調整することが示唆された。

The purpose of this study was to clarify the problems encountered by advanced lung cancer patients continuing outpatient chemotherapy and discuss appropriate nursing care. Interviews were conducted using a semi-structured questionnaire, involving seven patients with advanced lung cancer who continued two or more courses of outpatient chemotherapy. Analyses were conducted qualitatively and inductively. As the problems encountered by advanced lung cancer patients continuing outpatient chemotherapy, seven categories of pain caused by symptoms of adverse events due to anti-cancer agents, problems in daily living caused by physical symptoms, anxiety due to uncertainty regarding the effi ciency of chemotherapy, anxiety associated with cancer progression, anxiety over how life may end, difficulty in continuing outpatient chemotherapy, and diffi culty in playing a social role were extracted. In nursing care for advanced lung cancer patients continuing outpatient chemotherapy, the importance of the following was suggested: 1) to relieve physical pain caused by adverse events due to continuing anti-cancer agents, 2) provide mental support to ease anxiety over continuing treatment with unknown effi cacy, and 3) arrange an environment for patients to support their continuation of outpatient treatment.
