研究目的は，マーガレット・ニューマンの「健康の理論」のもと，造血幹細胞移植後に大腿骨頭壊死を併発した患者と看護師がパートナーとなって，パターン認識の過程を共に辿ることで，患者の病気体験にどのような変化が生まれるかを探究することであった。研究デザインは実践的看護研究のもとで，解釈学的・弁証法的方法を採用した。研究参加者は20-30 歳代患者2 名であった。データは面接の内容と研究者のジャーナルであった。 参加者らの病気体験は，「これまでの生き方の振り返り」「過去の苦悩の振り返り」「今も持ち続ける苦悩の表明」「人の支えへの感謝の表出」「苦しむ自分を認め前に進むことの表明」という5 つの局面を経て変化し，ニューマンが主張する‘ 自分自身を知ることによる人間の進化’ の様を示した。本支援は，移植後合併症が続くことによる困難な患者に生きる意味を見出すことにむけた支援方法として効果があるという示唆を得た。

The purpose of this study was to identify what changes occur in the experience of disease, when a nurse and a client who has undergone bone marrow transplantation (BMT) leading to avascular necrosis of the femoral head follow his or her awareness process together. The theoretical framework was Newman’s theory of health. A hermeneutic, dialectic approach was chosen in praxis research design. The participants consisted of 2 outpatients in their 20-30s. The data was the content of dialogues in the participant-researcher partnership and the researcher’s journals. The participants revealed fi ve transformational phases. These were: “refl ecting on one’s life”; “refl ecting on one’s past suffering”; “showing thanks for people’s support”; “showing one’s present suffering”; and “showing recognition of one’s suffering self and moving forward”. Their insights provided evidence of an expanding awareness and support for Newman’s theory. The fi ndings suggest that this nursing intervention helps outpatients who fi nd themselves in a diffi cult situation following transplantation.
